Notice of Privacy Practices

The Children's Clinic's Notice of Privacy Practices (NPP) Notice describes how we may use and disclose you or your child’s protected health information (PHI) to carry out treatment, payment, or health care operations, and for other purposes that are permitted or required by law. It also describes your rights to access and control you or your child’s protected health information. Protected Health Information is information about you or your child, including demographic information, that may identify you or your child and that relates to you or your child’s past, present, or future physical or mental health or condition and related health care services. Please review the Notice carefully. The Notice is available for download, but we will be happy to provide you a written copy, upon request.

You will be asked to sign what we call our HIPAA Consent Form at registration acknowledging that you have been offered and have access to the Notice.
