Ages and Stages Questionnaire (ASQ)

We believe that early detection of developmental delays and behavioral problems is crucial to better outcomes for children.  Monitoring child development is an important aspect of well child care.  This is consistent with recommendations from the American Academy of Pediatrics that include important developmental screening tools for all well care visits at 9, 18, and 24 months of age.  We use a developmental screening tool called the Ages and Stages Questionnaire (ASQ).  It allows us to follow your child’s development more closely and provide early intervention should it be needed.

Important Features of the ASQ Screening Tool:
  • Designed to screen children for developmental delays in the critical first 5 years of life
  • Reliable, accurate and well-studied
  • Designed for ease of use for parents.
  • 5 developmental areas: communication, gross motor, fine motor, problem-solving & personal-social
  • Involving the parents enhances the accuracy of the screening
  • Culturally sensitive and available in foreign languages (i.e. Spanish)

How to complete the form:
The "Ages & Stages" Developmental Screening Questionnaires will be given to you at the age appropriate well-child visit (9, 18, and 24 months of age) When scheduling appointments at these ages, we will give you an "arrival time" for your appointment that gives you sufficient time to complete this important questionnaire before being called back for your visit with the provider.

If you prefer, you may stop by our office in advance of your appointment and pick up a copy of the questionnaire to fill out at your convenience (prior to the date of your appointment).

After you have carefully completed the questionnaire, the results will be scored and reviewed by your child's nurse and provider.  Should the results of the screening indicate a potential developmental delay, we may refer your child to our partners in the community for additional evaluation and assistance.
